Dr. Roth is a wonderful and skilled doctor. He talks directly to me and makes me feel in control of my orthodontic outcome. The girls at the office are friendly and sweet and always make me feel welcome. They are quick to make emergency appointments and always have a friendly smile! I would highly recommend Dr. Roth and his team to anyone!

While in Chicago from Utah on 10.1.16 I had serious tooth pain & I was able to get a same day appt w/ Dr. Ossama Azzam. I needed a root canal & Dr. Azzam proceeded to do the procedure with great care & I was completely pain free for the entire duration (FIVE GOLD STARS Dr. AZZAM!!). MY PROBLEM: I paid with a credit card. Two months later no claim has been filed as promised so I can be reimbursed by my insurance & my two voicemail messages & two emails have gone unacknowledged and unanswered.
