Dr. Cook...A voice from the past... July 3,1984- Our family (my husband and 2 sons) on a month trip west...we are from North Carolina. Our second night was overnite in Fort Worth, TX. Our 10 year old son went down the slide into the pool. Slipped and broke his front tooth 3/4 off...Opened phone bk Called you...Didn't know city (and, of course...no GPS back then) YOU came for us, drove us to the office, ($75 later tooth fixed) Still there...even after braces~ great job....Thks again~God bless.
At first, his personality put me off a bit because he is so careful to try to put patients at ease that he tells jokes,compliments you on your smile, and even makes a design out of a paper towel for you to hold if you need to squeeze something. I didn't know what to think. But, after being his patient for about 20 years now, we are very comfortable with his approach. And, we never question him when he says something needs to be done. He has proved his honesty many times..
Dr. Givan has been my dentist and my daughter's dentist for years. Now he also treats my son in law and grandson. My 11 year old grandson broke two front teeth on Sunday. He was so upset. But Monday, Dr. Givan fixed those teeth and they look good as new. Dr. Givan and staff are just wonderful. .

They submitted charges to my insurance company without my authorization AND without performing the work!!! I Will NEVER use them again..
