3The Atlanta VA Healthcare system approved a request submitted by Dr.Russell on VA form 102570d dated 12/20/17 to perform dental care on my teeth 2.3.4 and 5 and teeth 18,19, 20, 21 and 22. This form clearly states DENTIST: NO PAYMENT WILL BE MADE UNLESS PRE-AUTHORIZED BY VA, The form further states \"The patient must not be requested to pay additional fees for these services.\" While I was in the chair, Dr. Russell decided to remove a functioning permanent bridge on teeth 22, 23, 24, 25, 26 and 27 which was not authorized as I was unaware of the teeth numbering system. She further demanded a payment of $650 for a flipper as a temporary replacement for these teeth, which was totally unnecessary and installed a temporary bridge. She also demanded payment of $950 to cover lab costs which I refused to pay. I sent several emails requesting she contact me to resolve these problems. She never responded and later informed me \"... that she let staff handle these matters.\" I had an appointment to have the permanent bridge installed and the night before, her office called to say the work would not be completed until paid in advance. Several times during this period, I requested her receptionist to request authorization for something to replace a bridge which the VA had previously recommended to be removed. No response whatsoever.
