6New procedure: Stupid health history forms REQUIRED by the office to be filled out from scratch annually is a ridiculous CYA waste of time exercise placed on patients. I lied and listed nothing. If they really want to know they can look at last years records and merely ask if anything needs updating, but I’m not going to spend 15 minutes on this nonsense for a dentist I have a 20 year history with. It’s insulting to patients just because some lawyer said it’s a good idea. You have my records
